I agree to be contacted about my application by phone call, text message, or email at the number and address I provided, including calls or texts sent using automated technology or an artificial or prerecorded voice. I understand that agreeing is not a condition of getting help or of receiving benefits, that message and data rates may apply, and that I can withdraw this permission at any time by asking.

I give my permission to Jason Muehlheausler (National Producer Number 19779611) of Muehl Group (National Producer Number 20605819) to help me with my Health Insurance Marketplace coverage. That help may include: completing a Marketplace application on my behalf, applying for the advance premium tax credit and cost-sharing reductions I may be eligible for, and enrolling me in a qualified health plan.
